Tuberculosis and phrenic nerve destruction.

چکیده
August 2007, Vol. 97, No. 8 SAMJ Phrenic nerve palsy (PNP) is often associated with Mycobacterium tuberculosis-related expansile pneumonia, possibly because of nerve entrapment by healing fibrosis. Mohan and Jayaswal1 postulated that pressure on the left phrenic nerve by an enlarged hilar lymph node caused unilateral diaphragmatic paralysis in a 6-year-old boy who developed PNP during the course of pulmonary tuberculosis (PTB). Gie et al.2 described 6 childhood cases of PTB complicated by unilateral PNP. The children did not recover on antituberculosis drugs and steroids, or in 1 case, after surgical decompression. Therefore the theory of glandular compression alone does not adequately account for PNP as a complication of PTB. Our case, in which the fate of the phrenic nerve was confirmed, provides strong evidence that destruction of the phrenic nerve by the tuberculous inflammatory process is involved in the pathogenesis of diaphragmatic paralysis, which may influence the clinical management of these children.

Apico-basal diameters of the lungs and mediastinal shift in phrenic nerve crush and pneumoperitoneum therapy; a study of eighty cases.
The objects of this paper are (a) to report on a study of mediastinal shift and apico-basal diameters of the lungs in a series of 80 patients undergoing phrenic nerve crush and pneumoperitoneum therapy for pulmonary tuberculosis: (b) to discuss the mechanism and therapeutic significance of the mediastinal shift. Although studies of mediastinal shift following phrenic nerve paralysis alone have ...
متن کاملA comparison of pneumotorax and phrenic nerve pulsy complications using the methods: supraclavicular nerve block, routine and ten degree cephalic route
Background: Supraclavicular nerve block is a method for upper limb surgery in Anesthesiology. In this method that is common for supraclavicular block, injection anesthesia on the first rib and it has complications such as pneumothorax, phrenic never pulsy, infection, intra spinal injection. In this study injection with a 10 degree cephalic side as compared with the first rib in order to decreas...
متن کاملThe Value of Phrenic Exairesis in the Treatment of Pulmonary Tuberculosis
nerve is found in the medial aspect of the muscle; very occasionally the nerve is in very close apposition to the brachial plexus and then it may lie on the lateral aspect of the muscle and may cross it only at the point of insertion of the muscle. The accessory fibres to the phrenic nerve follow the first part of the brachial plexus taking their origin usually from the fifth or sixth cervical ...
متن کاملA modified conception of phrenic nerve crush and pneumoperitoneum therapy.
The object of this paper is to present a modified view of the mode of action of phrenic nerve crush and pneumoperitoneum therapy in pulmonary tuberculosis. In a study of mediastinal shift in a series of 80 cases of phrenic nerve crush and pneumoperitoneum therapy (Fox, 1950) it was found that the average mediastinal shift to the left in the cases with right phrenic paralysis was 1.6 cm., and to...
متن کاملReinnervation of the diaphragm by the inferior laryngeal nerve to the phrenic nerve in ventilator-dependent tetraplegic patients with C3-5 damage
The aim of this study was to evaluate the feasibility of unilateral diaphragmatic reinnervation in humans by the inferior laryngeal nerve. This pilot study included chronically ventilated tetraplegic patients with destruction of phrenic nerve motoneurons. Five patients were included. They all had a high level of tetraplegia, with phrenic nerve motor neuron destruction. They were highly dependen...
